Application

This registration form should be printed, filled out and mailed to the school with a $50 processing fee.  If you don't already have an appointment to visit the school, we will call you to make one.

Your computer must have a PDF reading program, like Adobe Reader©, to open .pdf documents. You may download this free program by clicking the link provided.  You may have to re-open this website and page after the installation.
